How do you use the information you hold about me?

South Terrace Urology require the collection of personal information from individuals to enable the provision of health services and for related administrative purposes.

For example:

  • Assessment for provision of health care services;

  • To liaise with other health professionals, Medicare or your health fund;

  • In an emergency where your life is at risk and you cannot consent;

  • To maintain accurate medical records.

You hereby consent to South Terrace Urology:

Performing Administrative Functions

Using your Personal Information to perform administrative functions and activities in relation to our services. For example: to maintain accurate medical records.

Sharing with Others Involved with Your Care

Disclosing your Personal Information to other organisations and/or third parties involved in your care if it is necessary to be able to provide you with the service or information that you requested, and/or you have agreed to us providing it to the third party and/or it is already publicly available information.

For example: For use by a multidisciplinary treating team, disclosure to a medical expert (for medico-legal opinion), an insurer, a medical defence organisation, or a lawyer, solely for the purpose of addressing liability indemnity arrangements (such as reporting an adverse incident), or for the defence of anticipated or existing legal proceedings.

Sharing with Our Vendors

Disclosing the Personal Information we collect to third parties we engage to perform functions or provide products and services on our behalf such as performing medical procedures, providing data storage or processing credit card information.

Sharing for Billing Purposes

For billing purposes, sharing relevant aspects of your Personal Information with third parties such as Medicare, other providers directly involved in your care, or your insurer.

Responding to enquiries

Using your Personal Information to respond to enquiries you make regarding the services and/or the websites.

Complying with Laws

Using your Personal Information to comply with any applicable laws or if disclosure will prevent or lessen a serious or imminent threat to your life or health or the life of someone else.

Disclosing with your Consent

Disclosing your Personal Information, with your consent, to another person including any person you have authorised disclosure to or a person exercising an enduring power of attorney which has been provided to us.

Contacting you to ask you to provide feedback about the services provided.

South Terrace Urology will not use any personal information collected for any other purposes without the express permission of the individual unless required or authorised by law, or if it is for a directly related secondary purpose which is closely associated with the primary purpose.

We will never sell information about individuals to anyone else.
